Insurance & Payment options

How Payment Works

 At Arizona Autism Diagnostic Center, we are a private-pay practice and do not bill health insurance directly. Payment is collected at the time of service. We can provide a Superbill — a detailed document you can submit to your insurance company to request possible reimbursement. 

What is a Superbill?

A Superbill is a detailed receipt that includes all the necessary billing and diagnostic codes from your evaluation visit. This document allows you to submit a claim to your health insurance company for possible reimbursement of some or all of the out-of-pocket costs.  


It includes:

  • Provider information (name, NPI)
  • Dates of service
  • Procedure (CPT) and diagnosis (ICD) codes
  • Charges for each service rendered

How to Submit Your Superbill

  1. Confirm your insurance benefits before your appointment.
  2. After your evaluation, we’ll provide a Superbill.
  3. Contact your insurance carrier to learn their claim submission process.
  4. Submit the Superbill (and any required claim form) to your insurer either through their secure portal or by mail.
  5. Track your reimbursement.

Important Note

Insurance reimbursement is not guaranteed and is determined solely by your individual insurance plan.
