• Childhood Evaluation (In Person): $2,300

Arizona Autism Diagnostic Center is a private-pay practice and does not bill insurance directly. Payment is collected at the time of service. Upon request, we provide a superbill you can submit to your insurance carrier for possible out-of-network reimbursement.

A Superbill is a detailed receipt that includes all the necessary billing and diagnostic codes from your evaluation visit. This document allows you to submit a claim to your health insurance company for possible reimbursement of some or all of the out-of-pocket costs.  

It includes:

• Provider information (name, NPI)
• Dates of service
• Procedure (CPT) and diagnosis (ICD) codes
• Charges for each service rendered

1. Confirm your insurance benefits before your appointment.
2. After your evaluation, we’ll provide a Superbill.
3. Contact your insurance carrier to learn their claim submission process.
4. Submit the Superbill (and any required claim form) to your insurer either through their secure portal or by mail.
5. Track your reimbursement.

Insurance reimbursement is not guaranteed and depends entirely on your individual out-of-network benefits and policy terms.
